Idempotent GH/Jira Risk or Problem ticket keyed by project plus stable Charter variable_id. It stores current state, prior/current band including Unknown, evidence and provenance, freshness, severity, gating policy, assessment run ID, and lifecycle history. Repeated runs update the same concern; they do not create duplicates.
